Health Records Requirements

The College health forms, sent to all newly accepted students who have paid deposits, should be completed and returned to the Office of Health Services before the students' first semester. The College requires full-time students to submit documentation of a current physical and PPD (TB test), which can be recorded on the health form.

New York State requires all students to submit immunization records. They may submit an official immunization record from their health care provider or complete a MSMC Immunization Record; a health care practitioner must sign the immunization record. These records must be submitted before the beginning of the semester or session. For more information, see Health Services under Student Affairs. Students who fail to comply with these laws will be notified within 15 days of the first day of classes that they will be withdrawn from the College if they do not provide documentation by the deadline stated in the notification. Students who are withdrawn for not submitting this documentation will not be allowed to attend class for the remainder of the semester or session. Current course work will be assigned a Withdrawal (W) grade and the notation "Withdrawal/Immunizations" will be place on the student transcript. Students will not be allowed to register for the next semester or session until these state requirements have been satisfied.
