Application Procedures

Applicants must submit the following:

1. A completed application form.
2. Official transcripts from all institutions attended, undergraduate and graduate; student copies are not acceptable.
3. Two letters of recommendation. Students are encouraged to submit additional information or evidence to support their credentials.
4. A copy of the applicant's valid NYS RN license. 
5. A completed health form verifying current immunization as specified on the form.
